Blooper and BP...

"The patient had his blood pressure checked by Larry." 

hahaha, this one took the blooper prize today in the world of QA! 

actually dictated as "The patient had his blood pressure checked bilaterally"  ... which means of course that they strapped that contraption on both arms and got pressures.  Why would they do this? 

Don't ya'll know?  According to studies the potential value of an interarm difference as a simple clinical indicator of increased cardiovascular risk.  

Assessment of blood pressure in both arms is recommended by guidelines and should become a core component of initial blood pressure measurement in primary care.   However most GPs don't practice this.  Detection of an interarm difference should prompt consideration of further vascular assessment and aggressive management of risk factors.

In any event, the next time you see your GP (General Practitioner) ask him to take pressures in both arms, and see his reaction to your request unless of course there are contraindications specific to your health status, like for example having had a mastectomy.

What on God's earth could explain these behaviors? 

Well, sadly this patient was born with a chromosomal disorder (2q37 deletion) which in itself can cause physical dysmorphic characteristics, kidney and heart congential abnormalities, joint dislocations/scoliosis, other medical issues/illnesses, not to mention psychological disorders, mental retardation, autism, behavioral difficulties, bipolar disorder...and the list just grows on...


Folks, count your blessings is I can say.

and that one beats the reigning blunder of Hallucination Greer Blvd, which my boss informed me she sent to a client way back when she was an MT, she had "hal" in her auto correct to automatically spell out hallucination, so when the addy was Hal Greer Boulevard, well you get the gist of it...

Imagine My Surprise!



This morning when my alarm went off, and I had to get Isabella out of bed, I asked her if it was Friday.  I was thinking it was only Thursday, not looking forward to the day, but a quick check of my computer alerted me that it is Friday!  It couldn't come soon enough to suit me!

The complaints this week didn't last too long, everybody was served up with emails concerning their bloopers, and we moved on.  Our new In-House FQA liasion is stepping up to the plate, trying to deal with some serious issues between us and our offshore QA Department.  Thank goodness some good things are happening; its about frickin' time!

Shit Tuesday

I was not expecting today to be a day of complaints from clients. After being off for a week and my first day back, I would have liked a little more positive ending to my day.  However, supervisor informs me that not 1, but 2 accounts are complaining; one is thinking of pulling out altogether instead of using our services.  Oh well, its become quite a pain in the neck quite frankly.  They want perfection, let them go in search of it.  Good luck, and good riddance to bad rubbish who cannot dictate worth a shit anyway.  One of the complaints we got just gave us a rave review 2 weeks before.  How can they in two weeks' time now complain?  What did they do? get a couple of reports that had a mistake on them?  Oh yeah, one was a date that was dictated as July, not November, but we got blamed for that one, because we put what the doctor dictated as the date.  He could have been catching up on an old dictation, how would we know otherwise? 

Anyway, being in management and having to be over the QA department people and their lack of paying attention to details is always an accident waiting to happen.  Nobody is perfect, not in the world of quality control especially when humans are involved.  If these clients want perfection, then I suppose they can type their own documents and proof them too.  I don't care if I become part of an audit team or not, that's better than taking the heat for people who don't do their damn job.  Why would independent contracters be worried about error?  They don't get paid enough to worry or look stuff up like they should.  Perhaps the company should look at what they can do to improve things instead of cutting down their people, smacking their hands for making mistakes...stupid shit...yeah its Tuesday.  

What will tomorrow bring?  Who the hell knows...maybe I won't have a job to worry with tomorrow.

I have definitely had moments of stress this week.  Luckily nobody got hurt; however I am not vocally challenged when it comes to email...I will let 'er rip! Yep, they ain't paying me enough for this stress!  Its a constant battle with these ICs (independent contractors, not worth the powder and lead to blow them up most days) and employee FQAs reminding them of what they should already know and be doing on these accounts.  Do they read or follow instructions available to them? hell no...They would rather interrupt me on IM (instant mesenger) while I have enough on my plate with reviewing files and managing the workload flow.  Tonight it dragged on way too long, and at 9:30 p.m. I finally got done with what I had to do.  Fruck that shit! You know how this scenario goes.  Pete and RePete were sitting on a fence, Pete fell off, who was left? why Repeat (aka Re-Pete) of course!

Whose got booze? Somebody take me away from this madness!

Oh, did I mention? Isabella is at a pajama party tonight at her friend's house....which means, while the little one is away, mamaw can play! hahaha!

I am dog dead tired, folks....

Frankly, every time we have a Monday holiday, its hell the rest of the week. Its been nonstop madness the last two days.  We've had two new clients, an overflow of files, and tackling the onslaught of files has worn me out.  I worked from 7 to 7 two days in a row, today I just quit working at 5, and just said heck with it! I'm over it. 

"Little Queenie" (aka Isabella) and I walked down to check out the new Riverfront Tuesday the 5th, since we didn't even get close to it Monday night when Billy Ray Cyrus took the stage July Fourth; however Kenny Loggins and Blue Sky Riders, his new band took the stage.  Speaking of the night before, the Fourth, the police were not letting anybody pass into the Riverfront area due to over capacity as it was, even arresting those people trying to cross the tracks to get into the place.  Shameful.  However when a lot of the people left after the fireworks, they did let more people go in so at least they could hear Billy Ray sing. I just left, heck with that crowd.  I figured I could watch it on YouTube somewhere...LOL.

A neighborhood friend of mine walked over to us a little while before the concert was going to start Tuesday night (Kenny Loggins), and gave us her VIP tickets (that somebody just gave to her) to be seated up front next to the stage.  We were in the 2nd row, excellent seats, and the drinks were free! That tickled little Isabella as she was thirsty she said, so she helped herself to the freebies, enjoyed the concert and bedtime wasn't till nearly 11:30 p.m.  Getting up the next day was a bear! However the Blue Sky Riders were pretty good. 

Yes, work has been brutal this week.  Today, I was listening to a new client dictate, "The patient is doing well with her Horizontal Therapy treatments..." yeah...that's exactly what I was thinking....

Arthur-itis...

My right mouse hand is stiff and I can hardly bend my finger joints.  I believe old 'Arthur' has come to visit. I'm also having more musculoskeletal discomfort in my shoulders and neck area.  I think a trip in to the doc to get a little blood work done might be in order; also probably need to check my vitamin B levels. 

At least I don't have to type for 8 hours a day, that was hard to do.  I would have burning stinging palms, so red they looked as if they had been smacked by somebody.  I also have Raynaud's in the winter, the tips turn white and blue, and stay numb.  I know I'm immunocompromised with having hypothyroidism, and no intrinsic factor in my stomach to store vitamin B12 in my body.  Probably nothing a good shot of B12 couldn't fix on a weekly basis for a month or two.  

Getting back to work today after vacation literally wore me out.  I need a sponsor so I can quit this day job.  Any takers?

I saw this 637 minute file on the system today, and I thought to myself...

Yep, these doctors have to learn to turn off their recorders, and not leave them on for 10 hours after dictating a file...IF this actually has a patient dictation on it.

Yesterday I woke up to nearly 700 files on the system.  It didn't look bad Monday evening, but by the time Tuesday morning came, it was a madhouse!  However, with much diligence by my FQA staff and my maneuvering files around to compensate we actually pulled it off and got 'er done!  Somebody probably got this 637 minute file opened after half a day...LOL...luckily it wasn't one of our files, thank goodness!

Today it has been wild on the workfront. Stuck files, late files, the system down for an hour this evening. Its been one thing after another. So here I am plugging along, and all of a sudden this work computer just freezes up stiff as a board; I can't navigate anything...and this appears...

"DrWatson Postmortem Debugger has encountered a problem and needs to close. We are sorry for the inconvenience."

My first thought was where was Sherlock Holmes?...cracked me up! and of course the Postmortem addage...LOL..something died that's for sure!  Let me shoot this piece of shit Gateway work computer that the company gave me to use at home! Its not worth the powder and lead to blow it up!

Anyway, I figured out it probably wasn't a big whup...but

If you are using Internet Explorer, Outlook or any Application, you can get Dr .Watson Postmortem Debugger error. You can also observe drwtsn32.exe process running in the task manager.Dr.Watson crashes when other applications crashed.

What’s Dr.Watson? Is this a Virus?
Dr.Watson for Windows is a debugger that will be useful to debug and creates log files when a error occurs with in a program. Technical support people will analyze these log files and fix the errors. It is not a virus or a malware program.Dr.Watson creates log files at  C:\Documents and Settings\All Users\Application Data\Microsoft\Dr Watson.

It actually crashes some times frequently and gives the error as given below.

Why Dr.Watson crashes and give error?

Unfortunately, when you are browsing through Internet Explorer or working with any other program if that program crashes, then Dr.Watson program it self crashes and gives that error.

Does I really need this Dr.Watson  program?
No not really in fact Windows Error Reporting service also do the same as Dr.Watson, as you know you can send error reports to Microsoft through Windows Error  Reporting also.

What’s the Fix?
Fix to this problem is disabling Dr.Watson as we have alternative Windows Error Reporting available, we don’t require Dr.Watson .

Disabling Dr.Watson
1.Click Start>Run>type Regedit and click ok.
2.Navigate to HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows NT\Current Version
3.Select AeDebug in the right pane ,double click it and enter value 0.

disable-Dr.watson debugger_regsitry
4.Close registry editor, restart your computer.

You won’t get this error again in future.

If you have problem with access to registry, download the registry file released by Microsoft , run that registry file.

Here Comes The Sun...

Hello there.

Today, I was having to work double-duty to cover for another supervisor on her accounts and manage her people with workload as well.  No major catastrophes, but the day kind of ended up with me having to be on here till 7 or after.  Since I was on here at 7 a.m., that makes for a rather long day.  At 4:30ish, I'm getting ready to log off, Operations decides that they are having a crisis and needs an MT to type documents for this doctor because he is wanting his files back.  The account belongs to India in-house team, but they're in bed asleep, right?  Its the dead of the night over there.  About the same time Operations hit me up, the customer service Rep was hitting me up asking if I knew of a couple of MTs that could type up these documents, obviously late. and the doctor was bitchin'. 

First off, I am not in charge of the MT's (medical transcriptionists); I am in charge of FQA but that girl was out due to pregnancy; in fact she was going to hospital today to be checked and for possible admission.  Does anybody know what the plan is when the right hand woman is gone?  No...

I found two willing MTs, however unfamiliar with the accounts, so I pulled them typed samples out of Scribe, and helped them as best I could.  I told them to upload the documents to us (the FQA team).  One of the MTs was having a hard time with pediatric 'terminology' to include major medical terms.  He also talks a little fast to me.  I had FQA proof them, and I think the last file was uploaded at 5 p.m. sharp! hahaha, talk about a deadline.  There were no blanks left in these documents.  This was for a doctor's office in California.  We are good, yes we are!

I felt bad for the customer service Rep, even though my immediate supervisor told me I should not have gotten involved; its hard not to get involved when two people are asking you to help.  I called another supervisor for direction because somebody has to be in charge of the one whose out having a baby right? Well you would think so...I told my immediate supervisor that I would not do this again...! Operations only got involved too, both of us trying to help with the situation due to the supervisor of the MTs being out.  Somebody had better put some protocol in place on issues like this, because I refuse to overextend myself again, especially if the doctor complains about the files! Lets hope they were to his satisfaction.  We'll know for sure tomorrow.

and tomorrow ... the same chicka is off again, so I will be performing slave duty for her on top of my usual and customary responsibilities.  We always get slammed on Thursdays and Fridays...Maybe I'll call in sick tomorrow and wouldn't my immediate supervisor love that? yeah baby, yeah...Vacation bells are ringing!

Snit, Snot, Bitches...

Hormones are weird things in the workplace.  If my boss is having a extremely stressful day, she passes that tension on; otherwise, she's a real sweetheart.  Her and I go way back before this company ever merged, so we are familiar with each other.  I know her temperament, she knows mine.  We work well together.

By the time it was 5:30 p.m., she was apologizing to me for being "short and snippy," said she just had nobody she could take it out on, we laughed.  I know the pressure she is under, believe me, and I know the pressure I'll be under when she trapses off to India again for 8 weeks.  Yeah TWO Frickin' Months!!!!! 

Its been a few days of hooty wildness...snow, school closed, an 8-year-old driving me nuts, fighting and scratching, all while trying to do my job from home! You know how girls can be if they are getting on each others nerves, cooped up because of the snow. School had better be in session tomorrow! If that's not enough, my car died yesterday, deader than 4 o'clock.  It won't even turn over, and I haven't even dealt with that yet.  I suppose the weekend is right around the corner, maybe I can bribe a friend to check under the hood and see what is wrong. 

If I make one resolution this year, its to get a damn car! I'm tired of this constant worry with this vehicle.  She's been good to me though for many years now, but like all good things they must end, right? If you'd like to donate to the poor appalachian need-a-car fund, feel free.

Do I look a little stressed?

Yep, that was me at the start of my workday.

Actually I knew yesterday that today was going to be murder on the FQA homefront. Since I am team leader (aka lower class management...lol) I had more jobs than people to get 'er done, and quite frankly it was getting worse by the minute. It seems the recording lines for the doctors were busy, busy, busy! this weekend.  We had tons of work to get out. 

Some of my FQA people can do certain clients, some can not, therefore complicating my dilemma, and furthermore the same sick chicka yesterday evening, let me know she wasn't working today, and client who has a 4-hour turn around and a 6-hour turn around TX client would need coverage.  I was not doing it again tonight, yesterday I had no volunteers...just they wait until they want vacation time! ha!

By 3 p.m., thanks to my diligent staff and much coercion by myself for them to work over if need be, we had this thing licked and we didn't have to work over! and I have coverage for TX and stat files.  Damn I'm good.

Anybody Got An Aspirin?

my head hurts.

and its no wonder. We have a new FQA person, whose picture reminds me of "I Dream of Jeanie" with some garb she had on when viewing her picture on Yahoo Instant Messenger. She is blonde, or fake blonde, however I'm trying not to hold that against her...LOL...but I find myself wondering if the light is on and nobody's home? She does not work in the office, which at this point is a good thing for her as I might have choked her several times over the course of three days now. I don't deal with stupid well...I try to keep perspective...she's just new...she's just new...yeah right.

She technically has had more problems than anybody. Her voice file system (Scribe) was not working; if that wasn't enough then she couldn't upload her documents. Yesterday in 8 hours she did seven reports. Seven, mind you. Even on a bad day with bad dictators, FQA people would upload 4x more documents. Hers were not hard dictators. Come to find out, the Scribe error was because she was not doing it right ('user error'), not because of a technical issue with the program. I personally went over this with her as did a customer care representative. However, the uploading issue is still hanging around today. Customer Care Service felt maybe it was her server, so she contacted her server. She said she got angry at her server AT&T DSL and hung up on them. It was thought that perhaps during a heavy period of people online that her server was failing to let her upload.  However, my suspicion is that its 'user' error again.

It has been two days of relentless issues with nobody else but her, our operating system is fine, as nobody else is having issues but her. She is obviously of a mindset that I have nothing else to do I suppose but fill in the blanks on her reports that she cannot get, or upload documents for her throughout the day. I have countless other things to do all day than to deal specifically with her all day long from the time she starts her shift till she finishes it. I am ready to set her on fire at this point.

Not to mention her productivity today on the easiest stuff I had was only 21 reports; easily one should do 70 on this particular account, most do 100. The reports at most were only 2 minutes long, and were urology reports; supposedly something she has experience doing.  She also was initially hired to help out with a new client coming on that is going to require around the clock turn around (TAT) times, even on the weekends, and now she is not wanting to work weekends.  At this point, she cannot get through one day without assistance.  If she cannot work alone, of what purpose is she?

Tomorrow she is on my ignore list, sink or swim, and if I don't see any improvement in her workflow or the amount of work she is proofing, she will be whacked off....

Can you say.....next!

Wait...this just in my email from "I Dream Of Jeannie."

Dear _____..."I just got off the phone with Time Warner, so my internet will be road runner DSL. I am hoping that will be better for me to upload and not have anymore problems. I would like to get your thoughts on this switch before I do it."

okay correct me here, folks, but didn't she say she got off the phone and her internet will be RoadRunner?  then she wants thoughts BEFORE she does it?? (by the way, RoadRunner is cable internet, not DSL).

I rest my case. 

Can I trade that aspirin for a Valium?

Who wants to bet that she will still have upload problems even if she changes servers?

but unlike blue balls up there, this patient had epidydmitis which did not clear up with a first round of antibiotics! He was given another round of antibiotics and told to follow up with his family physician!

where one touch of my IM sends a request halfway across the world, a response back, and a thank you from me, with a message back that says, "No need to thank me, I am here to serve you."

Really? I was taken aback a little.  Been a while since I heard that one! too bad the guys in the US aren't that eager to please! ha! However my next immediate thought was that I could get used to that! LOL ... Does that mean I can order up a back massage and a catered lunch? Oh, that might be asking a little too much and a little hard to get done from India! yikes!

I do love it that these people are so eager to serve! that's because they are taking over the medical transcription business.

This past week was a nightmare on the management homefront for a newbie like me, if it wasn't email issues, it was scheduling difficulties, taking responsibility for allocating work to 8 different people, learning the procedures, navigating the software...yada yada...I can tell you I was not prepared to step into the shoes of someone who does the job of 3 people, and who is lucky she is trapsing halfway around the world in India, cause if she were close by, I would have choked her! ..and several times! LOL

I let her have my rant one night in an email, raving about my lunacy of a day and how inept I felt I was not trained to do this job in her absence, la-te-da, and the fact that I was not expecting the responsibility I got slapped with, and that my paycheck had better match my stress level or else! LOL I told her that if I got custody of my granddaughter here in a week or so, that I did not have time for two 24/7 jobs! and allocating on the weekends too!

What a pain in the rawwrrr! There were two days there I just about told them to take that job and shove it!  I was ready to bail, had my stuff walking out the door! stressed out to the max.  I was ready to drink and smoke too byGod! Especially on Friday, somebody's internet was down, they were only going to work till noon; another one's computer crashed on them, we got bombarded with work over what we normally get, it was crazy! my email was acting up AGAIN!!!! and it was a juggling act all day just to get the work allocated to the right people to keep the customer reps happy and the clients happy! Sheesh! Even though I did the best I could at allocating, there was not enough people to cover that mad workload, so I worked from 7 to 7 myself, but vowed that would be the last day I'd do that.

Today, Monday, was a piece of cake compared to Friday!  I am just now feeling a little comfortable with all I need to do.  Being in the production business doing medical transcription reports, we have TAT (turn around time) mostly of 24 hours.  That means from the time its called in till the time its out is 24 hours, only a few clients give us 48 hours.  My DDS TAT times were exceeding 100 hours.  Critey! that's what happens when you do not have enough QA people to do a particular client and the workload for that one particular client is just massive!  The US in charge of production (my boss for nearly 12 years) and who is now a part of this large corporation looked at me today and said, "I do not want you having to proof at all, your job is now management, and we have the people, and the resources ... use them."  He was very nice to me as he knows I am trying to keep everything lined out, and will even dive in myself and proof if need be.  I told him I would have no trouble doing that! just show me the money and give me the extra hands! I had forgotten that being around all the old office people is kinda fun, actually!

and how was your week? weekend?  Sheesh where I have been? Working like a slave I tellyuns!

Go Ahead, YAWN!

I don't know about ya'll but some days even the coffee don't perk me up.  I'm dead at my desk here this afternoon, and I had planned on trying to work over to make up a few hours due to the issues we had with our latest system upgrade here first thing Monday morning. 

Its still screwed up, resulting in our QA query not getting our regular work quota, and since the QA workload is messed up now for the last two days, I have been typing more instead of my usual QA duties; that always wears me out. 

Not to mention, I was still typing my last report, when my two grandkids descended upon me yesterday, brought by the MIL (mother-in-law) for nearly 4 1/2 hours so she could get her peace and quiet after having them all day long, and I was worn out from entertaining them. They were hooty wild.  

Whatever the combination, to include all this boring white stuff that's been flying around and accumulating all day and the cold temps, even in this drafty house...I am retiring to my bed early, turning off the phone ringer, and dying wrapped up in my blankie.

the keyboard issue...

I don't think too much about my keyboard until it sticks, or tears up, and I've been known to wear the key letters off a few keyboards in my line of work as a transcriptionist over a period of 10 years.  Family who have to hunt and peck are out of luck when it comes to looking for letters on my keys! I've also seen the newer development of more ergonomically designed keyboards over the years to reduce possible injuries related to the wrists from carpal tunnel syndrome from occupations such as mine.   

I do spend a lot of time at my keyboard typing, not so much in the last few months, but when you type on one for 8 hours in your job for many years, or you repetitively use one everyday, you should really be concerned about what kind of keyboard you are using. When I bought my HP it came with a Microsoft Natural keyboard which was split.  I loved it!  Every since the design came out many years ago with split right and left hands I started using this kind of keyboard.  Even switching from an older split keyboard to my new Microsoft one took adjusting to. 

When my keyboard stopped working due to probably all those potato chips and cracker crums, and not to mention a few spills accidentally, I was subjected to using a straight keyboard and I thought I would die! I could not type on that thing if my life depended on it.  It is whatever you get used to, but beware! You might be causing yourself problems down the road.  I know on a regular keyboard my fingers seemed really squished together, and talk about having to slow down to type! geeze Louise! I guess if I ever got a laptop I would have to either attach my own keyboard cause I don't think they even come with a split keyboard!

Anyway, if you haven't tried a softer, more ergonomic keyboard such as a natural split keyboard, I suggest you try one, and in the long run your hands/wrists will thank you! I know I've been typing like a madwoman for 10 years now, and I have been injury-free of carpal tunnel signs or symptoms.  Be preventative.  Be smart.

Today in the land of "twats-and-whatnots"

(that's short for urology) I just about fell out of my chair laffing on this one.

"The patient complains of difficulty with urination and perennial pain."

 You mean its seasonal? hahaha! (should be 'perineal' relating to between the legs)

Ole Frank here could suffer some perennial pain if she whacks him with that book she's trying to read!
