today's anatomy lesson...

I had a doctor today dictate ... "Limb length from anterior superior iliac spine (ASIS) to medial malleoli is _? _ inches."  He is so foreign that I could not make out what he said the measurement was.   I'm trying to wrap my head what the MT had originally put in as this value; and at the same time I'm looking at my own leg thinking...that can't be right.
Okay, bloggers, lets do the math on this (like yuns care, right?) but hey! this just one of the many things I may have to research in a day's time cause you cannot trust these onshore or offshore typists or editors to know anatomy.  So...just where is your anterior superior iliac spine or your medial malleolus, you ask? the pic pretty much says it all...true-length-leg-measurement
Anatomically, from the top of your hip bone move a tad over towards midline and run an imaginary line down to your medial ankle ...
I pulled up old reports from the database and found that some MTs had rather laughable values typed in old reports already uploaded to the client. From info I found online, even a 12 year old with normal weight of 90 some pounds and around 5 feet tall, have 29 or 30 inch measurements from ASIS to ankle, so one can assume an adult would be at least over 30 inches!  (Oh boy, I'm sure the client won't be happy to receive those reports back where the measurements were totally off making us look like boobs!)
I ask you, Bloggers, from observation, do you think normal adults would only have 12 inches from ASIS to ankle? or even 20 inches? I think not! that is some short-assed midget people! .... I wished these MTs/Editors would use their noodles...for criteys sakes!
..and that concludes your anatomy lesson *class dismissed!*

Who we all take for granted ... the garbologists! Its funny every year in my little town, the garbage crew will leave a little "card" in an envelope in the lid of the garbage can with a little note this time of year.  I just found mine today, when I took my garbage out.  It always cracks me up what they come up with every year.

Speech Wrecks~


On any given day, you never know what you'll find when you are QA'ing reports.  Sometimes the Editors fail to correct the speech recognition engine which results in such errors... 


  • He reassures me that he has kept my family in his purse and I thanked him for that and inquires about my ill father. (prayers)
  • The patient today has a rash as well seems to be improving on off of work.  (with Aquaphor)
  • He also had back surgery approximately year and a half ago and arches type surgery for his left knee in 2006.  (arthroscopic)
  • Outside temperatures are quite cold with the windshields... (wind chills, lol)

Speaking of wind chills, is it cold in your neck of the woods?

Current weather here says its 19 degrees; but feels more like 14.  A heat wave after last night's 7 degrees!  Brrrrrr!

Workday funny ...

Well, I had a doctor who is foreign dictating a examination on a patient applying for disability benefits.  They list allegations claimed by the patient, history, makes an assessment of how many hours the patient can sit or stand in an 8 hour workday, how much weight the pt could lift, and/or any other restriction when it comes to work...

and then he says..."She will have difficulty with any vocation requiring that she lay flat."

Come again? Yep that's what he said I listened to it twice!

tell me Bloggers ... What kind of job would require somebody to lay flat on their back?

....

Okay don't answer that ... made me laugh...hahaha!

All In a Day's Work ...

I am proofing a patient's oncology report this past Friday ... doctor is dictating and I'm reading ..."We again reviewed available information, discussed the questionable solitary metastasis in the brain, and reviewed various clinical scenarios with the possible of patient having a legal metastasis" ...

wait a minute there...

that ain't right...I knew immediately what it was...

Obviously the speech recognition engine spit out what it heard but MT failed to catch/edit it.

Actually it was dictated as ... with the possibility of patient having oligometastases.  That's the term right there ... and that folks...is why I still have a job!

and in case yuns don't know (assuming you might want to know)...according to the Google Gods...

Oligometastases is the state in which the patient shows distant relapse in only a limited number of regions. Local therapy such as surgery, radiotherapy and radiofrequency ablation for the relapsed sites could thus improve patient's survival.
